Winning With TYpe 1 Diabetes By Learning How To Use Continuous Glucose Monitoring CGM

Thank you for your interest in registering to attend our first "double-header" Sugar Surfing Workshop to be held in Longview, Texas on June 23, 2018. Stephen W. Ponder, MD FAAP CDE will be your primary speaker and facilitator for this event.  In addition, preceding Dr. Ponder's Workshop, Cassie Moffitt, MSN, RN, CDE will speak about Alternate Caregiver training for families in which caregivers such as grandparents and extended family or friends provide care to your T1D child.

 

Participants may register for the full program, which includes lunch, or for the afternoon only, to participate in Dr. Ponder's Sugar Surfing Workshop.  We are excited to present our first full day educational diabetes program!

Dr. Steve Ponder has been recognized for his impact on diabetes care as the 2018 Diabetes Educator of the Year by the American Association of Diabetes Educators (AADE). This organization represents more than 14,000 Certified Diabetes Educators located throughout the USA.

The Sugar Surfing workshop will take place at Pine Tree Church of Christ Fellowship Hall, 3221 Dundee Road, Longview, Texas.

 

Admission for the FULL DAY event, which includes lunch is $40 for an individual, $55 for two, and $75 per family of 3 or more.  Admission for the AFTERNOON ONLY session is $25 for an individual, $35 for two, and $50 per family of 3 or more. Lunch is not provided for afternoon only participants.  Child care is NOT provided at this event.

The Sugar Surfing Workshop will begin promptly at 1:00 pm and ends at 4:00pm (includes 20 minute intermission).
